Programmer's Reference manual updated

There are quite a few fixes and corrections and mostly work to keep  
the manual synchronized with the sources.

There are also a few new sections:

Method queries, although it doesn't cover every possible one: http://

#methods, #roles, and #roleIndices come to mind for the latter.

I'll also need to expand the Traits section on the new Trait  
operations when that system is ready for use.

Comments are invited - I know this thing is usable but it's not  
nearly as good as it could be.

And don't nudge me in the direction of writing a book before the  
language is useful. ;)
